Annual Longevity Physical
A 60–90 minute comprehensive exam with Dr. DeMarco—focused entirely on you. We’ll review your data, address your concerns, and set meaningful goals for the year ahead. This is a wellness visit the way it was meant to be: strategic, personalized, and designed to optimize your health—not just “check the boxes.”
Unlimited Primary Care Visits
In-person or virtual visits for preventive care, chronic conditions, and urgent needs—whenever you need them, all included. Whether it’s a complex issue or something that requires regular follow-ups, we’ll take the time to make sure you fully understand your health concerns and treatment plan—even if that means weekly visits for a period of time.
Chronic Disease Management
Ongoing care based on the best evidence, focused on finding and addressing the root cause of your health issues. We help with conditions like high blood pressure, prediabetes and diabetes, high cholesterol, thyroid problems, anxiety, depression, ADHD, asthma, COPD, and autoimmune diseases—whether your condition is stable or you’re still in the process of getting it under control. Our goal is not just to manage symptoms, but to help you improve—and when possible, reverse—the condition.
Sick Visits & Same-Day Messaging
When you’re sick, pressed for time, or need care urgently, you’ll see Dr. DeMarco—same day or next day. Many issues can be handled quickly through virtual visits, getting you what you need so you can keep moving when it matters. No urgent care waiting rooms, no impersonal telehealth with a hurried provider who doesn’t know you—just fast, personal care from your own physician, who knows your history and goals.
Clinical Secure Messaging
Convenient, HIPAA-compliant messaging that gives you direct access to Dr. DeMarco for non-urgent questions, follow-ups, plan clarifications, or quick updates on your progress. Like being able to text your doctor—securely and promptly—so you can get the support you need without unnecessary delays.
Advanced Care Coordination
When a serious illness or urgent health issue arises, patients shouldn’t have to navigate it alone. Care is coordinated from start to finish—referrals arranged when possible, communication maintained with specialists, and oversight provided before, during, and after treatment. With limited time to speak with most specialists, an experienced physician advocate ensures the right questions are asked, recommendations are interpreted, and next steps are clear. The result: less stress, less confusion, and more peace of mind when it matters most.

DexaFit Core Testing
Your annual membership includes 2 total tests per year (a combination of DEXA, VO₂ Max, and RMR) — a value of up to $500. For longevity, Dr. DeMarco recommends a DEXA Body Composition Scan and a VO₂ Max assessment. For those who cannot walk on a treadmill or ride an exercise bike, we recommend a DEXA and RMR, or 2 DEXA scans/year.
Members also receive 35% off any additional testing if desired and clinically appropriate.
DEXA Body Scan — A scale only shows total weight, without revealing what’s changing inside your body. DEXA is a gold-standard, 3-compartment model that separates bone, fat, and muscle. It shows whether weight loss (or gain) is coming from the safest place — helping you track progress with precision and uncover hidden risks like visceral fat around vital organs.
RMR Test — Ideal for those on a fat loss journey. Accurately calculates caloric needs, fat-to-carb burn ratio, and metabolic flexibility to make weight loss safe, sustainable, and personalized.
VO₂ Max Test — More than a fitness test, VO₂ Max is the single best measure of how long and how well you’ll live. It reveals how your body uses oxygen, your activity capacity, and your heart-rate zones for training. Think of it as your “independence score” — the ability to stay active and capable well into later years.
Vitality Blueprint Lab Analysis
Most lab reports are filled with numbers and ranges that don’t mean much unless you’re a doctor. The Vitality Blueprint takes that same raw data and transforms it into something clear, practical, and personal — so you actually understand what’s going on in your body and what to do about it.

Additional Notes
A one-time $99 administration fee will be collected with your first payment.
For monthly plans, payments will be automatically charged each month to the payment method on file, beginning on your membership start date.
For those choosing the First-Year Prepaid Special Offer, the next monthly payment will be due on the anniversary of your start date.
Memberships require a 6-month minimum commitment. After that period, cancellations may be made with 30 days’ written notice.
